Whether you are new to HPSM or just getting started on your healthcare journey, we want to make it easy for you to use your benefits. These quick tips can help you get started!

Your HPSM Member ID Card is your proof of HPSM membership. You need this card to use most of your benefits. Bring it with you to all your healthcare appointments and the pharmacy.
Using the Member Portal can help you get things done anytime from anywhere without having to call HPSM. With just a few clicks on your computer, tablet or smartphone, you can:
- Select or change your primary care provider (PCP)
- Update your address and phone number
- Request a new HPSM member ID card
- Check your immunization records
- And more
To sign up for the Member Portal, all you need is a computer or mobile device with the internet and a few minutes. Get started!
Get a health assessment
CareAdvantage members: take the Health Risk Assessment (HRA)
The HRA is a set of questions about your health offered to CareAdvantage and select Medi-Cal child members. HPSM’s Member Assessment Unit offers the survey to new CareAdvantage members by phone. The call takes only 20 minutes. Your answers are used to create a care plan that covers your medicines, doctors visits, exercise and more. Both your HRA and care plan are confidential.
The HRA can be taken in two ways:
- Download the form, complete it and return to HPSM
- Call 1-888-783-3035 (toll free) or 650-616-5035, Monday to Friday between 8:00 a.m. and 5:00 p.m.
HPSM Medi-Cal members: get an Initial Health Assessment (IHA)
Are you a new HPSM member? Or have you not seen your primary care provider (PCP) for your first healthvisit? If so, call your PCP to make an appointment today for your IHA! At this first health visit, your PCP will:
- Give you a check-up
- Ask you questions about your health
- Make sure you get the care, medicines and vaccines you need to be healthy
- Give you information to help you stay healthy
Choose a PCP and make an appointment
As your main provider, your PCP leads your care team. They know your medical history, your medicines and which treatments have worked for you.
Find out how to choose a PCP at www.hpsm.org/choose-pcp-tips. To choose a PCP you can:
- Log into the Member Portal
- Call HPSM (either Member Services or the CareAdvantage Unit)
- Scan the QR code or visit www.hpsm.org/find-a-provider to find a provider
Do not choose a PCP marked “EPO” — it means “established patients only” and that the PCP is not taking new patients.
Choose a dentist and make an appointment
- Go to our Find a Provider webpage
- Enter your information in sections 1 and 2
- Select “Dental” and click “Search”
- Choose your dentist
- Call them to schedule a visit: tell them your insurance is HPSM Dental
