Last April, the Health Plan of San Mateo (HPSM) launched four Primary Care Provider Grants, each aiming to bolster primary care teams in different ways. Read about the outcomes of the first year of the program, as well as important changes to come in year two.

Year 1: Our First Year in Review
In the Primary Care Provider Grants first year, HPSM has:
- Received 40 grant applications from 15 primary care organizations
- Approved 31 grants comprising of $5.5 million in grant funds
- Disbursed $1.8 million in grant funds to 14 primary care organizations
Through the four grant categories, HPSM approved funding for:
- 9 Doctor of Medicine(MD)/Doctor of Osteopathic Medicine (DO) positions
- 6 Nurse Practitioner/Physician Assistant positions
- 12 Medical Assistant positions
- 1 Scribe position
- 15 language incentives
- 5 down payment assistance incentives
- 9 sabbaticals
- 3 workflow improvement projects
- 2 AI documentation pilots
- 1 eConsult integration project
Year 2: What’s Coming Next
In the second year of the Primary Care Provider Grants program, we want to highlight three updates:
- New Payment Option: To increase the uptake of the Core Team Stabilization grant among independent practices, independent practices can now choose between two payment options:
- Receive grant funds in one lump sum up front for recruitment and retention; OR
- Receive grant award at two points of time after PCP or MA hire:
- Two and three years from hire date for MAs.
- Three and five years from hire date for PCPs.
- Application Requirement: As of May 2026, organizations are required to apply for the Core Team Stabilization grant no later than six months from the hire date of a new PCP or MA to be eligible for grant funds.
- Primary Care ACT collaboration: Later this year, primary care organizations participating in Primary Care ACT coaching and technical assistance will have opportunities to discover how to make the most of available grant funding. Both programs aim to advance primary care by strengthening primary care team capabilities and capacity.
