CareAdvantage members have access to a large and diverse provider network that includes:

  • Over 500 Primary Care Providers (PCPs)
  • Over 5,000 Specialty Care Providers
  • Over 1,300 Ancillary Health Providers
  • 8 Hospitals
  • 5 Community Health Centers
  • Chain and local pharmacies

Where Can I Get Care?

The CareAdvantage provider network includes doctors and medical facilities in San Mateo County and San Francisco. CareAdvantage covers your health care throughout the United States. If you need care outside of those areas, CareAdvantage may cover needed care you get from out-of-network providers.

Telemedicine

Doctor's visits by phone or video anytime

Many providers can give you advice over the phone or see you by video.  Telephone and video visits are fully covered by HPSM. Call your primary care provider (PCP) to check if they provide telephone or video visits. 

If you do not have a PCP, or if your PCP does not offer phone or video appointments, you may be able to use Teladoc®, one of HPSM's newest service partners. Learn more

Continuity of Care

If you have recently joined CareAdvantage, you can keep seeing the doctors you go to now for a certain amount of time. You can keep your current providers from the time you enroll for up to 12 months for Medicare and Medi-Cal services if you meet all of the criteria described in the CareAdvantage D-SNP Member Handbook. This is called continuity of care.

After the continuity of care period ends, you will need to see CareAdvantage network providers unless we make an agreement with your out-of-network doctor.

Getting care outside of the U.S.

You have coverage up to $25,000 anywhere in the world for emergency care and transportation services.

Search for a provider

Use the online provider search form to find the latest information for in-network primary care providers (PCP), specialists, pharmacies, clinics or hospitals. This directory is updated weekly.  You may need a referral from your PCP before making an appointment with a specialist for non-emergency care. Call your PCP to find out if a referral is required.

  •  Search results include links to interactive maps and directions
  •  Print or save the information for individual providers
  •  Weekly updates ensure you get the most accurate results

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Benefits and co-pays may change on January 1 of each year.
